<p>神經醯胺（Ceramide）是由皮膚表皮最外角質層細胞，與細胞之間所組成的高濃度脂質（脂肪）分子，是一種本身就存在於肌膚角質層內的「生理脂質」，是表皮上的角質層的重要成分，具有親水性與親油性的化學特性。</p>

<h2 class="wp-block-heading" id="h--1">形成肌膚保護屏障</h2>



<p>角質層細胞間脂質的主要成分有神經醯胺、脂肪酸及膽固醇三種脂質，這些脂質在對抗外部環境傷害中扮演重要角色，當中神經醯胺存在人體皮膚角質層的脂質中含有50％以上，和膽固醇及脂肪酸同步組合成特別保護力的結構，形成一個防護屏障，防止肌膚水分流失。</p>

<p>相比玻尿酸、A醇、維他命C，神經醯胺特別適合維持皮膚健康的重要循環。神經醯胺的保濕能力高，加上它能保護皮膚，穩定肌膚的屏障功能，對抗空氣污染及壓力造成的皮脂過度分泌，能有效保護表皮肌膚不會被環境刺激所傷害，讓皮膚表皮層更加平滑細緻，達到抗衰老效果。<br></p>

<p>神經醯胺屬於天然型保濕成分，本身不容易過敏，不容易產生嚴重的副作用，人體肌膚也會自體產生神經醯胺來保護肌膚結構，所以適合每一種膚質，痘痘肌及易出油的肌膚都非常很適合，尤其對乾燥、敏感的肌膚特別有幫助，也適合年長肌膚和長期日曬傷害肌膚。</p>

<h2 class="wp-block-heading" id="h--5">為甚麼需要補充神經醯胺？</h2>

<p>神經醯胺本身存在肌膚當中，為甚麼需要另外塗抹？紫外線、空氣污染、氣溫上升、年齡增長等情況，容易使皮膚原先的神經醯胺陸續下滑。肌膚中含有的神經醯胺，從25至28歲開始，逐漸流失大約40%。步入40歲開始，更會失去大約60%以上。隨住肌膚老化，肌膚上神經醯胺逐漸減少，肌膚屏障容易變得更加脆弱，造成乾燥、粗糙、皺紋、敏感、泛紅和脫皮。</p>

<h2 class="wp-block-heading" id="h-vs">玻尿酸 VS 神經醯胺</h2>



<p>玻尿酸屬於「保濕因子」，神經醯胺屬於「脂質」，最大的差別在於玻尿酸是補水，神經醯胺則是防禦水分流失、穩定肌膚狀態。<br><br>玻尿酸容易被吸收且最不易致敏，補水快且抓水力強，但若要使其完全發揮效用，必須有其他非身體原本的水份使其吸收。例如剛洗完臉，趁皮膚有水分時使用添加玻尿酸的保養品，使其抓水；接續以乳液或乳霜等油脂成分建立起一道鎖水屏障，避免玻尿酸直接接觸空氣而水分蒸發，以達到有效保濕。<br><br>但假若肌膚角質層的防禦力已出現問題時，只使用玻尿酸加強保濕是不夠的，可補充神經醯胺強化角質防禦，阻擋水分蒸發。</p>

<h2 class="wp-block-heading" id="h-"><strong>適當劑量魚油有助減低心血管疾病風險</strong></h2>

<p>2019年，一篇發表於新英格蘭醫學期刊《The New England Journal of Medicine》的論文發現，使用以EPA成分為主的高劑量魚油（每天4公克），對於原本有心血管疾病的受試者，可降低25%的續發心血管風險，包括減少心血管疾病導致之死亡、缺血性心臟病、非致命性中風發作等。<br></p>

<p><br>而美國心臟協會（AHA）建議所有成年人每周至少吃含高Omega-3脂肪酸的魚類（每次約3安士）2次，以攝入足夠分量的Omega-3脂肪酸。對於慢性心臟病患者，AHA建議每天補充DHA及EPA合共1克。</p>





<p><br>許多研究都顯示，魚油的效果與吃的劑量有關，劑量越高效果越好。不過，吃太多魚油會影響胃腸的消化與蠕動，產生噁心感、肚子不適而難以下嚥等反應，所以高血脂患者如需補充較高劑量，則應尋求專業醫護人員指導。而有服用薄血藥人士更需注意，由於Omega-3脂肪酸對血小板有抑制作用，應避免食用高劑量的魚油產品。<br></p>

<h2 class="wp-block-heading" id="h--1"><strong>驗出<strong>魚油丸</strong>含有致癌污染物</strong></h2>

<p>經過消委會測試後，發現逾9成半樣，即一共24款樣本均驗出含有有毒的氯丙二醇成分。據動物實驗顯示，長期每日過量攝入會損害腎臟、中樞神經及影響雄性生殖系統。其中GNC超級魚油 (180粒)、楓之寶高濃度強心奧米加3、美然康高效DHA魚油丸3款樣本檢出量超出歐盟每公斤2500微克的上限。同時，楓之寶高濃度強心奧米加3檢出過量抗氧化劑BHT。</p>

<p>另外，消委會發現6款樣本驗出含有基因致癌物PAHs，而苯並[a]芘可作為食物中具可能致癌性PAH的指標，在高溫烹調帶脂肪的食物中較常見，屬基因致癌物，應盡量避免攝入。</p>





<h4 class="wp-block-heading" id="h-6">涉及的6款樣本包括：<br></h4>



<ul><li>&#8211; L&amp;F阿拉斯加深海魚油</li><li>&#8211; 日本命力增強版強腦素</li><li>&#8211; 美然康高效 DHA 魚油丸</li><li>&#8211; 澳至尊兒童健腦魚油益智補腦配方</li><li>&#8211; 日本阪聖孕婦專用DHA</li><li>&#8211; Nutronic 全孕期奧米加3<br></li></ul>



<p>其中檢出PAH總量最高的為「Nutronic」孕婦魚油丸，檢出的4種PAH總和為每公斤54.0微克，苯並[a]芘含量為每公斤12.7微克，超出歐盟規定上限每公斤10.0微克。</p>





<h2 class="wp-block-heading" id="h--2"><strong>魚油補充劑非仙丹</strong></h2>

<p>雖然奧米加3脂肪酸在維持眼睛、腦部和心臟健康方面的作用已廣為人知，但對於認知能力、記憶、智力、情緒、肥胖、皮膚和過敏方面的問題，則未有足夠的數據證實任何DHA或EPA劑量會有明顯效用。</p>

<p>2021年有研究分析孕婦或哺乳期婦女食用魚油補充劑與其子女認知能力之間的關係，結果發現所有受評估的認知能力參數與兒童服用DHA或EPA補充劑沒有明顯關係。營養師則認為，父母應為孩子建立健康的飲食習慣，不要偏食，只要兒童從均衡的飲食中攝取足夠的營養以促進成長和發育，就不需要特別補充奧米加3脂肪酸。</p>





<p>香港醫學會代表婦產科專科醫生凌霄志表示，有醫學文獻指出懷孕期間服用奧米加3脂肪酸（包括補充劑或以食物例如魚類、海產作補充）的婦女當中，早產（尤其是&lt;34週）的情況有所減少；亦有可能降低圍產期死亡、新生兒護理入院及低出生體重嬰兒的風險。</p>

<p>魚油為脂溶性營養素，建議要飯後吃或是隨餐吃效果最佳，成人1日攝取1000毫克就可以，孕婦1日則可以攝取2000毫克，按照台灣衛生福利部食品藥物管理署建議，1日勿超過2克</p>

<p>孕婦懷孕期間攝取Omega-3對於胎兒發育有很大幫助，特別在妊娠晚期(懷孕28周至40周)，DHA會大量聚集於腦部及視網膜，有研究指出，懷孕期間攝取適量Omega-3，幼兒在2歲時的手眼協調力測驗分數較高。</p>

<p>「乾眼症」是眼科門診相當常見的疾病之一；指的是眼睛淚液分泌量不足夠或是分佈不均勻或淚液過度蒸發，造成淚液無法適當的保持眼球表面的濕潤所造成。有研究指出，魚油有助減少乾眼症狀和角膜螢光染色檢查數值，並增加了淚膜裂散時間（tear film break-up time, BUT）和Schirmer淚液分泌試驗值。</p>

<p>根據美國心臟協會的建議，一星期吃2份魚肉，有助於降低罹患心腦血管疾病的風險，例如三文魚、沙丁魚、鯖魚等油性魚類中，含有豐富的Omega-3脂肪酸，研究指出罹患冠心病會使認知能力下降的風險增加45%。</p>





<h2 class="wp-block-heading" id="h--4">魚油對藥物的影響</h2>

<p>魚油有抗血小板功能，所以魚油丸如與具抗血小板功能或抗凝血的藥物或食物補充劑（如銀杏、人參、薑、黨參等）同時服用，理論上可能增加出血的風險。此外，由於魚油丸可能會輕微降低血壓，故與降血壓藥物同時服用或會增加其降血壓的藥效和風險。以下四類人士服用魚油前需特別注意，向專業醫生查詢自己是否適合服用魚油:</p>





<h2 class="wp-block-heading" id="h-1">1. 正在服食心臟藥人士</h2>

<p>魚油有抗凝血作用，若有在服用阿斯匹靈、抗凝血劑等藥物，務必告知醫生有吃魚油的習慣，讓醫生監測凝血功能，避免腦出血問題發生。</p>





<h2 class="wp-block-heading" id="h-2">2. 規劃要進行手術之人士</h2>

<p>進行手術期間，比較容易出血，所以準備要做手術的人，要暫時停止服用魚油，避免手術進行時引致大量出血的情況發生。</p>





<h2 class="wp-block-heading" id="h-3">3. 孕婦或授乳媽咪</h2>

<p>由於深海魚油可能含有重金屬、二噁英（Dioxin）等有害物質，過量服用魚油或會對身體有害，故兒童、孕婦及授乳婦女不宜進食過多。同樣，脂肪較多的魚類也只可適量進食，以免攝入大量甲基汞（Methylmercury）、PCB、二噁英及其他污染物。美國環保署建議孕婦及授乳婦女每周進食脂肪較多魚類的限量為12安士。此類人士如欲服用魚油補充劑，應先諮詢醫生或註冊營養師的意見。</p>





<h2 class="wp-block-heading" id="h-4">4. 高血脂人士</h2>

<p>雖然有研究顯示，奧米加3脂肪酸能有助降低體內三酸甘油脂，並輕微改善俗稱好膽固醇的高密度脂蛋白（High Density Lipoprotein，簡稱HDL）水平。然而，如患高血脂人士服用高於每天3克奧米加3脂肪酸（DHA及EPA），則有可能提升俗稱壞膽固醇的低密度脂蛋白（Low Density Lipoprotein，簡稱LDL）水平。高劑量的奧米加3脂肪酸（DHA及EPA）有降低血壓的作用，惟必須在醫生指導下服用。</p>
