<h2 class="wp-block-heading" id="h-65">年滿65歲即符合醫療券資格</h2>



<p>年滿65歲並持有有效香港身份證或入境事務處發出的《豁免登記證明書》的長者，即符合資格領取，及使用醫療券支付私營基層醫療服務的費用。<br><br>現時每名合資格長者每年可獲發$2000醫療券金額，而累積金額上限為$8000。每年的醫療券金額會於該年1月1日存入醫療券使用者的戶口內。</p>

<p>政府於2022年的施政報告中，宣布優化長者醫療券計劃，包括：<br>— 容許夫妻共用醫療券<br>— 擴大適用範圍至「認可醫療專業註冊計劃」的聽力學家、營養師、臨床心理學家和言語治療師<br>— 擴大適用範圍至經專業評定並提供的醫療設備，例如助聽器等<br>— 把每年$2000的醫療券增至$2500。</p>

<h2 class="wp-block-heading" id="h-2500">長者醫療券獎賞先導計劃：醫療券加碼至$2500</h2>

<h2 class="wp-block-heading" id="h--1">夫妻共用長者醫療券</h2>



<p>夫妻共用醫療券的原則是，若對方用完自己的金額，有需要就可以用配偶的醫療券。長者及配偶只需於任何一方使用醫療券時，一同親身到參與長者醫療券計劃的醫療服務提供者的執業地點，出示雙方的香港身份證或入境事務處發出的《豁免登記證明書》，聲明雙方的配偶關係和同意共用醫療券，醫療服務提供者便可經系統把他們的醫療券戶口連結，讓長者可以由同日起接受私營基層醫療服務後，當長者本人醫療券戶口內的餘額耗盡，選擇使用其配偶戶口內的醫療券餘額，直至任何一方提出書面請求選擇退出為止。<br><br>只要長者配偶雙方的醫療券戶口成功連結後，當長者本人醫療券戶口內的餘額耗盡，即可選擇使用其配偶戶口內的醫療券餘額，使用的金額不設上限，但不得超過該次所獲醫療服務的費用。但是，視光服務配額以每名長者計算，所以每名合資格長者可用於視光服務的醫療券金額上限為每兩年$2000維持不變，不可共用。<br><br>如已登記共用醫療券的配偶離世，離世配偶戶口內的醫療券餘額不可以由在世長者使用。使用已離世配偶的醫療券，可能會被控詐騙等罪行及須負上刑事責任。</p>

<h2 class="wp-block-heading"><strong>長者醫療券</strong>適用地方：</h2>

<p><strong>問：領取綜援、傷殘津貼的65歲或以上的長者，是否合資格成為醫療券使用者?</strong><br>答：是。65歲或以上的本港居民都合乎資格。<br><br><strong>問：65歲長者可否使用醫療券支付社康護士的費用?</strong><br>答：視乎社康護士受聘於哪種機構。如社康護士由私營機構聘用，合資格人士便可使用醫療券支付此項服務費用，其原則是避免政府雙重補貼。<br><br><strong>問：醫療券可否用於私家醫院的急症室服務?</strong><br>答：可以。若此急症室服務是門診，不涉及住院服務，原則是可以使用醫療券支付服務費用。<br><br><strong>問：醫療券可否用於日間手術程序，例如白內障手術或内窺鏡檢查服務?</strong><br>答：不可以。長者醫療券計劃旨在加強對長者的基層醫療服務，並不適用於日間手術程序。</p>

<p>肥胖會增加患慢性病的風險，例如糖尿病、心臟心血管病。當中擁有「大肚腩」的肥胖人士就更加高危，因為研究指出，中央肥胖的人患上二型糖尿病的風險，比沒有中央肥胖的人士高3倍。想成功減磅，可以參加地區康健中心的瑜伽、健身、打拳班，預防9大慢性病。</p>

<p>有研究指出，中央肥胖的男士和女士，患上二型糖尿病風險，分別比其他人高出5.1倍和11.1倍。體重指數（BMI）雖被廣泛用於衡量肥胖程度，但未能有效反映身體的脂肪分佈。中央肥胖俗稱「大肚腩」，即腹部積聚過量脂肪，與整體脂肪過多的致病風險同樣高。研究指出腰圍可反映腹部脂肪積存的程度，從而評估個人的患病風險。<br><br>長期高熱量飲食、缺乏運動、過量飲酒等不良生活習慣，容易積聚內臟脂肪。當內臟脂肪積聚得愈多，患上二型糖尿病的風險愈高。</p>

<p>要解決中央肥胖問題，減重是有效的方法。恆常運動及改善飲食習慣均十分重要。</p>

<p>南佛羅里達大學（University of South Florida）的研究團隊針對 6820 名平均年齡為 53 歲受試者的睡眠數據進行調查，他們需提前自我報告個人睡眠特徵和心臟病史，而當中有633人手腕上佩戴了一個研究設備，睡眠數據更準確及清晰。研究人員就各方面睡眠數據作分析:</p>

<p>研究人員利用美國資料庫《MIDUS》（Midlife Development in the U.S.)的資料，分析以上6項睡眠品質指標，結合受試者過往被診斷為心血管疾病的醫療記錄，結果發現每增加一種睡眠障礙問題，罹患心臟病風險就會增加54%，而當中通過自我報告和研究設備共同提供睡眠數據的人，罹患心臟病風險更上升至141%。</p>

<p>睡眠不足與瘦素有關。瘦素是一種人體所分泌的物質，會抑制食慾， 降低脂肪細胞的脂肪儲存。另外飢餓素刺激人腦中樞，增加食慾。每次脂肪細胞吸收過多熱量時，就會分泌瘦素到血中，並通過血腦屏障（Blood-Brain Barrier），進入下視丘，增進飽足感、降低食慾、促進肌肉代謝。瘦素在夜晚分泌最為旺盛，因此捱夜或睡不好，會使瘦素下降，令人容易暴飲暴食而增加體重。</p>

<p>瘦素在凌晨12時至3時分泌最旺盛。研究發現，一日只睡5小時的人，瘦素比起一日睡滿8小時的人，少了16％。<br><br>另外，跟睡眠時間有7至9小時的人相比：<br>睡眠時間6小時的人，肥胖率會提升23%<br>睡眠時間5小時的人，肥胖率會提升50%<br>睡眠時間4小時以下的人，肥胖率高達73%<br><br>因此最簡單的減肥法，是養成晚上12前入睡，至少睡滿6小時。</p>

<p>於2021年JAMA Network Open發表的一份研究<a href="https://jamanetwork.com/journals/jamanetworkopen/fullarticle/2776441" target="_blank" rel="noreferrer noopener">《太極拳或運動對失眠老年人睡眠的影響》</a>中，證實太極拳可以改善失眠。<br><br>研究人員將320名60歲或以上，有失眠問題的成年人分為3組，分別為對照組（110名）、運動組（105名）和太極拳組（105名）。<br><br>對照組不進行任何干預，運動組參加為期12星期的常規訓練，包括快走和肌肉訓練，太極拳組就參加為期12星期的24式太極拳訓練計劃。<br><br>研究結果發現，運動組及太極拳組的成年人，睡眠效率均有所改善，包括入睡後的醒來時間和覺醒次數，而且效果可達24個月。跟對照組比起來，運動組的成年人每晚入睡後平均醒來時間減少了17分鐘，太極拳組的成年人每晚醒來時間則減少了13分鐘。</p>

<p>城市人壓力大，長期於高壓環境下生活，精神壓力上升，容易影響睡眠，躺在床上久久不能入睡﹑扎醒﹑難以進入深度睡眠，睡眠質素降低，長遠而言更會影響精神健康，及身體抵抗力。想不靠藥物以改善睡眠質素，不妨參考以下5件事。</p>





<h3 class="wp-block-heading" id="h-1"><strong><strong><strong>1. 培養固定睡覺時間</strong></strong></strong></h3>

<p>不少打工仔習慣周末睡午覺，這樣做其實會影響生理時鐘，因此不論周末、假日，都應該於平日的睡眠時間休息，這樣才會養成容易入眠的習慣。為了維持瘦素的分泌，最理想是養成晚上12前入睡。</p>

<p>調查發現，睡前一小時使用不同的電子產品，包括：電腦、電子遊戲機或手機等，都會令人難以入睡，皆因電子產品所發出的光線會刺激腦部，令腦部難以放鬆，上床休息時，就不要看手機了！</p>

<p>手機、電腦所發出的少量燈光，都足以影響大家睡眠，如果開燈睡覺，影響之大可想而知。燈光亦會擾亂令身體盡快入睡的褪黑激素荷爾蒙，因此，還是關燈吧！</p>

<p>做運動好處多多，有研究指出，每星期進行4次帶氧運動，睡眠質素顯著改善，不過要留意，晚上做運動，最少要隔幾小時，以免過度活躍，反而影響睡眠。</p>

<p>最新發表的《財政預算案》中，財政司司長陳茂波指吸煙危害健康，建議即時將每支香煙的煙草稅調高６角，並按同等比例提高其他煙草產品的稅率，為減少吸煙和戒煙提供誘因。政府的目標是2025年本港吸煙人口，由目前的9.5%降至7.8%。吸煙可導致心血管疾病、影響生育能力、增加患癌風險，究竟有甚麼戒煙貼士？又有哪些機構及康健中心提供戒煙的支援服務？現時全港哪裡有中醫針灸的戒煙服務？</p>

<p>多項醫學研究均顯示，每兩名長期吸煙者中，便有一名因吸煙而提早死亡。<br><br>煙草含有逾7,000種化學物質，其中很多是帶有毒性及帶放射性，當中超過70種是致癌物。煙草主要的三大殺手，包括：<strong>焦油</strong>、<strong>尼古丁</strong>及<strong>一氧化碳</strong>。</p>

<h3 class="wp-block-heading">5. <strong>孕婦</strong>流產風險高10倍</h3>



<p>吸煙可使母體內的血氧含量降低，胎盤中的血氧含量也會隨之減少。女性吸煙者的自然流產機會，較非吸煙者高出十倍。<br><br>吸煙孕婦在懷孕及分娩時，有較大機會出現併發症，例如增加胎盤位置移向子宮口，造成出血、胎盤早期剝離的機會，也會減少母乳的分泌。<br><br>孕婦吸煙亦會造成嬰兒體重下降大約8安士，懷孕時吸煙愈多，嬰兒出生時體重就愈輕。這些嬰兒不僅體質弱，且出生一年內患上嚴重疾病的危險性會更大、猝死率更高。吸煙孕婦的嬰兒，死亡率比非吸煙孕婦的嬰兒高出三分之一。<br><br>懷孕期間吸煙，增加嬰兒在一歲前患上哮喘的風險高達2.8倍，亦會引起嬰兒患腸絞痛、顎裂之風險。另外亦會影響嬰兒的生理成長和智力發展，如身高較矮、閱讀和數學能力較弱。</p>

<p>可利用戒煙貼、戒煙香口膠、戒煙糖、戒煙吸劑或藥物瓦倫尼克林 (Varenicline) 輔助戒煙。<br><br>使用這些戒煙輔助藥物能紓緩煙癮，一般使用期為3至6個月。<br><br>除了西藥外，近年中醫針灸戒煙亦開始備受重視。針灸能減低戒斷症狀，52周後戒煙成功率高達34%。</p>

<p>現時香港各區的地區康健中心，核心團隊由護士、物理治療師、職業治療師、營養師、藥劑師及社工等組成，而且提供戒煙輔導。<br><br>地區康健中心亦定期舉辦不同的運動課程，透過培養運動習慣，幫助戒煙人士轉移視線、有助減壓、降低過往因吸煙而造成的慢性病風險。</p>

<p>過往我們習慣患病才去看醫生，很可能拖到病情嚴重時才求醫，或是發現時已錯過及早治療的黃金期。去年底政府推出「基層醫療健康藍圖」，推廣「一人一家庭醫生」計劃，透過社區推廣「預防勝於治療」的概念。</p>

<h2 class="wp-block-heading" id="h-">基層醫療健康藍圖</h2>



<p><br>基層醫療健康專員彭飛舟醫生，早前接受<a href="https://www.facebook.com/watch/?v=649964820254005" target="_blank" rel="noreferrer noopener">訪問</a>時，介紹了《基層醫療健康藍圖》，提到以往醫療系統注重治療，即有病才去醫治的概念，而《基層醫療健康藍圖》就是想建立一個以預防為重、以社區為本的基層醫療制度，推廣「預防勝於治療」。<br><br>計劃於社區建立一個家庭醫生系統，讓市民可以於社區內，找到適合自己的醫生。這些家庭醫生，亦可能會於地區康健中心當助理，幫助市民預防疾病及提高健康狀態。</p>

<h2 class="wp-block-heading" id="h--1">基層醫療服務對象</h2>



<p>基層醫療的服務對象是所有香港市民，並不是只服務低收入家庭。整個醫療改革是透過家庭醫生、地區康健中心等，提高市民的預防意識，同時於社區內為市民提供醫療支援。<br><br>政府預計於今年第二、第三季推出「慢性疾病共同治理計劃」，資助市民透過「一人一家庭醫生」和跨專業公私營協作模式，讓市民在私營醫療服務界別診斷和管理慢性疾病（尤其是高血壓和糖尿病）。<br><br>彭飛舟醫生指出，香港現時大概只有23%的市民是有固定的家庭醫生，但研究顯示，有固定家庭醫生的人，相比沒有家庭醫生的人，健康狀態更好。</p>

<h2 class="wp-block-heading" id="h--2">助市民建立健康計劃表</h2>



<p><br>政府計劃先建立家庭醫生系統，再透過「慢性疾病共同治理計劃」作為介入點。高血壓和糖尿病這兩大疾病，常見於40多歲後。市民可透過篩查，知道自己是否患有這些慢性病。<br><br>政府資助市民，過了某指定年齡後，可透過地區康健中心，建立健康計劃表，隨後就會推出慢性疾病的高血壓和糖尿病的篩查。</p>

<h2 class="wp-block-heading" id="h--3">政府提供針對性資助<br></h2>



<p id="h--4"><br>醫務衞生局轄下設立策略採購統籌處，透過統籌處跟業界相關人士一起討論，哪個資助計劃是最有效可行。要取得資助的醫生，必先登記成為基層醫療計劃的家庭醫生。<br><br>現時香港有一萬五千多名醫生，部分是於醫院、醫管局及公營系統工作。其餘的醫生，有的是專科醫生，而可以提供基層醫療服務的醫生，估計有四至六千名醫生。<br><br>「慢性疾病共同治理計劃」提供針對性的資助，給予配對好的市民及醫生，鼓勵醫生持續跟進病人的病情。<br><br>彭飛舟醫生認為，家庭醫生的概念本身是一種關係，當一個家庭，跟一位家庭醫生建立長久的關係，當成是「家庭成員」之一。現時我們有「家庭醫學」這專科，讓醫生有更好的溝通技巧，跟病人建立更好的關係。</p>

<p id="h--4">政府計劃於未來優化「醫健通」系統，達到「一人一健康戶口」的概念，令醫療服務的提供者，更清楚病人的健康狀況，以加強保障醫療服務使用者，保證質素及提升醫療標準，並提高醫療過程的協調及連貫性，尤其是社區基層醫療和公立醫院之間的雙向轉介。</p>

<h2 class="wp-block-heading" id="h--5">推廣疫苗接種及癌症篩查</h2>



<p><br><br>對於「小病在社區」的概念，彭飛舟醫生認為「小病」的定義，是常見的慢性疾病，而慢性疾病又分「前期」。例如有前期糖尿病症狀的人當中，有10%的人會變成糖尿病患者，如果能於前期糖尿病時已及早改善身體狀況，加上醫生適當的介入，將能大大提高身體的健康狀態。<br><br>政府亦計劃成立基層醫療署，重整及統一服務，透過社區推廣基層醫療，例如注射預防針、進行大腸癌篩查等。</p>

<h2 class="wp-block-heading" id="h--6">逐步加強牙醫中醫師服務</h2>



<p id="h--8"><br>彭飛舟醫生表示，我們應善用社區內的資源，除了醫生及護士外，基層醫療健康計劃亦需要社工、物理治療師、營養師等專業人士的加入。<br><br>繼成立家庭醫生系統後，亦會逐步加強牙醫、中醫師、社區藥劑師、精神科醫生及其他基層醫療專業人員，在提供基層醫療服務中的角色。</p>
